Services available at the organisation
The Palliative Care Team presently consists of 1.3 WTE Consultants in palliative medicine, 4.5 specialist palliative care nurses and 1.1 lung cancer
nurse specialists. The Palliative Care Team meets regularly with healthcare professionals from other disciplines to plan and deliver care for patients with palliative care needs.
The team has formal medical links with the Marie Curie Centre and Northern Ireland Hospice
Specialist Palliative Care Units. Reasons for referral
to the Team include:
- Pain management.
- Other symptom management e.g. nausea
, vomiting, dyspnoea, terminal restlessness. - Emotional/psychological support for the patient and/or their family to enable them to explore issues and concerns related to advanced illness, and in addition facilitate communication between the patient/family and ward team.
- To facilitate discussions surrounding ethical dilemmas.
- Advice regarding complex discharge planning e.g. “home to die”.
- To act as a resource regarding other specialist services.
- Assessment for transfer to a Specialist Palliative Care Unit.
- Outpatient appointment with a Palliative Medicine Consultant.
