Hospital palliative care team. Referrals are taken for all adult patients inpatients and outpatients. Patients are also seen in Sperrin Room day chemotherapy unit. We see both cancer and non cancer patients with palliative care needs.

General Referal Statement:
The palliative care team accepts referrals for all adult patients who wish to be referred (ie with their consent)

Mode of Referral : for Service User
Referral must be by a healthcare professional. Patients may self refer for a MacMillan grant or information.

Mode of Referral : for Healthcare Professional
Referrals are made on referral form and sent to team secretary or if urgent are made by bleaping palliative care team.
