Transforming Cancer Follow Up

The Transforming Cancer Follow-up and survivorship programme is seeking to transform cancer follow up across Northern Ireland in line with the National Cancer survivorship initiative, to move away from routine hospital follow up, with its ‘one size fits all’ approach, to more individually tailored after care arrangements.
